Amanda oporta - Community dementia network

Amanda Oporta, CDP, CDCM, SSD is a Certified Dementia Practitioner, Certified Dementia Care Manager with a background in Social work, Program planning and Medical Administration. Amanda has over 20 years of experience in Dementia Care and is the state lead Dementia Friendly Indiana Communities. She has been Positive Approach to Care (PAC) certified since 2017. In her role she spearheads the development of dementia friendly communities across the state, advocates for better care and support for people living with dementia and oversees several programs, including the Dementia Friendly Care Partner Development Program and Dementia Friendly Business Trainings.

Dayna thompson - dementia education

Dayna Thompson, M.S., Ed.S., LMHC is a Licensed Mental Health Counselor who has been with IU Health since 2006. She has been trained in Positive Approach to Care (PAC) since 2016. Dayna is responsible for client consultation and counseling, community and professional education, outreach, professional collaboration and consultation, and service coordination and development. She is the Dementia Friendly Bloomington liaison.

Diana matthews - Rural dementia network

Diana Matthews, CDP has worked in the senior living and long-term care industries for over 15 years. As a Certified Dementia Practitioner who is serving as the Program Director at the Rural Dementia Network (RDN) through the ADRS, she is responsible for client consultation and counseling, community and professional education, outreach, professional collaboration and consultation, and service coordination and development. She also manages the grant that sustains the RDN.

She is the Dementia Friendly liaison for Orange County.

Shelly Gilbert, CDP has been working with families of persons living with dementia for more than 10 years. She is a Certified Dementia Practitioner and currently works as the Network Director at the Rural Dementia Network of the ADRS. She is responsible for client consultation and counseling, community and professional education, outreach, professional collaboration and consultation, and service coordination and development. She is the Dementia Friendly liaison for Lawrence County.
